STEP 3

DOCUMENTS NEEDED:

The following 7 documents are due on Thursday before the first day of class. (Submit documents via email: info@thecnatraining.com)

  1. Proof of Education (high school diploma, GED, transcript etc)
  2. Negative Tuberculosis Test Results (PPD, blood results or chest X-ray report)
  3. Copy of the COVID shot record
  4. BLS PROVIDER card - (non expired, issued by American Heart Association)
  5. U.S Government-issued Photo Identification with Signature

This identification must be current (non-expired) and must have both a photograph and a signature. The examples listed do not exclude other forms of ID that match the policy. Acceptable forms of PRIMARY ID include but are not limited to:

  • Driver’s License
  • Passport
  • Texas ID Card

6. A Second Form of Photo Identification

The name on the second form of ID must match the Primary ID and must include a picture of the candidate that matches their current state. The examples listed above do not exclude other forms of ID that match the policy.

Acceptable forms of secondary ID include but are not limited to:

  • School ID card
  • Facility ID card
  • Credit card/debit card: Not accepted for Pro-proctor Exams
  • Library Card

 ID EXCEPTIONS:

  • A combination of a High school ID with photo and signature AND Birth Certificate with names on both documents matching is acceptable.
